You only need to pay $50 at the time of service
If you use a 10% coupon, you can save 10% off the entire visit.
With proper ID, including valid address, you can set up a $25 per month payment plan with our billing department with 2.5% interest per month.

After the visit you can still file the claim with your worker’s comp. insurance and get reimbursed.
